Marine Accident Investigation Branch (MAIB)
As part of the UK’s Department for Transport, the MAIB investigates marine accidents involving UK vessels worldwide and all vessels in UK territorial waters.

Confidential Reporting Programme for Maritime (CHIRP)
CHIRP aims to contribute to the enhancement of maritime safety worldwide by providing a totally independent confidential (not anonymous) reporting system for all individuals employed in or associated with these industries.
From the website, you can download archive copies of their ‘Maritime Feedback’ publication, to read the safety lessons learned from their confidential reporting programme.
Alert! is a Nautical Institute project, sponsored by The Lloyd’s Register Foundation, that aims to improve awareness of the human element in the maritime industry. Whilst the programme has now finished there are many useful safety related references still valid on this site.
